The bottom of page 16 is the formula for Ho with a place for refraction. Refraction is found in the table, bottom left of page 17, with a longer version on page 104 Not sure if we use forms in this book or not... other than something like table 5-2a, bottom right of page 19, which does have a line for refraction.
