I *love* the SP-104 form. It's about the most elegant work-form design I've seen.
I love it so much I used it as the basis for a personal version, with things moved around a little so that I had room for some reminders and rules.
And then I made variants for Lat-by-LAN and Lat-by-Polaris, using the same basic structure, all laid out in the same 2-up (half-sheet) format.
I checked with StarPath and they had no objection to my sharing. Open to any feedback or improvement ideas.

Afterthought... I have these set up in "publisher" in a format that works great for printing. Can also print to a PDF, but since the source workforms are copyrighted by StarPath, I want to be sensitive to their rights and priorities.
So if anyone wants printable versions of these, lets work with StarPath to see how (or if) they want to handle that.

David, thank you for a great course...enjoying it immensely. Regarding form SP-104, I wasn't able to find the meaning/reference to what the capital T can capital A mean in the boxes above box 6. When calculating 'a', I'm using the HO-MO-TO catch phrase, and it works for me...is the capital T and A, in that box, just another visual clue for determining 'towards' or 'away'? Thanks.

Somewhere in the instructions it should say that for those without a favorite jingle to use (ie Ho Mo To), you can just choose the letter next to the larger value.

David, thanks for the clarification...helpful. Final confirming question on Ho vs Hc...when we compute a it is always the bigger number from the smaller, doesn't matter if it is Ho-Hc or Hc-Ho? That's where the T and A come in, correct?

That is correct. Subtract smaller from larger and give that result a label.
