We just posted an interesting note Can exceptional behavior be considered ordinary?
The issue of what constitutes "ordinary practice" as specified in Rule 2a is an ongoing point of discussion in admiralty court cases. The new edition of Farwells has a full chapter reviewing examples that were and were not considered ordinary. This example, however, is not from that book but from another one.
